​INSTRUCTIONS FOR COMPLETING THE CLAIM FORM
CLASS ACTION REGARDING STRIP SEARCHES PRIOR TO THE FIRST VIDEO-APPEARANCE

​ Please read these important instructions BEFORE you begin filling out the form.

​Class members:
We remind you that this class action is only for people who meet all the following criteria:
1) taken to the Rivière-des-Prairies or Montreal (Bordeaux) detention centres for an initial video-appearance
2) between October 11, 2016 and March 20, 2020
3) strip searched at the detention centre prior to the video-appearance and
4) released the same day by the Court, with or without conditions.

This does not include strip searches:
- for which the person was not released on the same day (including situations where conditions of release were not met on the same day)
- performed by police forces.

Fill out one form for all the strip searches you have experienced that are covered by this class action.

​Deadline:
You must submit your claim to the Administrator BEFORE OCTOBER 5, 2024.

Claim:
The Court has appointed the firm of Raymond Chabot to administer the claim process. The Administrator will work jointly with the ASRSQ.

You will be required to declare that all information provided in the Claim Form is true and accurate, under penalty of perjury.

If you need assistance in completing the claim form, contact the Claims Administrator.

Confirmation:
If you submit your Claim Form electronically, you should note the case number that will appear on the screen after you click "Submit." This number is confirmation that your claim has been received by the Administrator.

Payment of Compensation:
Following the judgment rendered by the Superior Court of Quebec on March 22, 2024, you will receive $2,000 for each eligible search you have suffered. Within 30 days of submitting a completed claim form, you will receive a decision indicating the number of eligible searches you have suffered.

Your compensation will be paid by cheque sent by mail. Therefore, you must notify the Administrator of any change of address.
